Terms and Conditions
1. The FreeStyle Libre Subscription Plan is for deliveries to Singapore addresses only.
2. The FreeStyle Libre Subscription Plan requires a valid credit card to be securely saved to your account profile. This credit card will be charged upon your next upcoming order date. Orders will not be accepted, fulfilled or shipped if the credit card fails for any reason whatsoever, including expiration and insufficient funds.
3. An order will be placed for you automatically every 25 days, unless you choose to change or cancel your subscription plan. For example, if you made your first order on 1 December 2021, your next order will be on 26 December 2021. Your subscription will continue unless it is cancelled.
4. Your credit card will automatically be charged when your product is delivered for subsequent purchases after the first purchase, unless you choose to change or cancel your subscription plan.
5. You may only subscribe to one subscription plan at any point in time. Should you choose to upgrade or downgrade your existing plan, please cancel your existing plan and add the new plan to cart before checking out.
6. For subscription plan cancellations, you must cancel your plan at least 3 working days before your next order occurrence date. Cancellations or changes will take effect on the next order occurrence cycle. Please refer to the subscription FAQ above on how you can cancel or change your subscription plan.
7. Not valid for use in conjunction with other FreeStyle Libre promotions or offers.
8. Abbott reserves the right to change pricing and offers at any time. Subscribers will be notified of such changes at least 1 billing cycle in advance through email notifications.
9. Except for any changes to offers and pricing set out in point #8 above where prior notice will be given to you, Abbott reserves the sole and absolute right to withdraw, add or vary any term relating to your subscription from time to time without giving any prior notice to you. You will be bound by the terms and conditions so amended.
10. An email reminder will be sent to you 7 days before your next order occurrence date. You will receive your electronic bill via email after your order has been automatically placed and your credit card will automatically be charged when your product has been delivered to you.
11. If for any reason payment by way of your credit card is declined (e.g. insufficient funds, exceeded credit limit, incorrect details, card expiration etc.), your order will not be automatically placed. Your plan will be placed “On Hold” for 72 hours from the order occurrence date, during which you may make changes to your credit card details to keep your planned order. Otherwise, your order will continue to be on pause.
12. You agree that Abbott’s decision on all matters or disputes relating to your subscription and any matters ancillary thereto (including without limitation payment, delivery, promotions or eligibility requirements) are final and conclusive and binding on you.
13. You hereby agree and consent to:
a) Abbott using and disclosing your personal data for the purposes of processing your subscription application and to provide you with the products you have requested; and
b) The terms of our Privacy Policy at https://freestylelibre.com.sg/privacy-policy as amended from time to time and the terms of our Privacy Policy are incorporated in the Terms and Conditions for the FreeStyle Libre Subscription Plan by reference.

Please read and understand the following information related to the use of the FreeStyle Libre Flash Glucose Monitoring System and consult your healthcare provider for any clarifications or queries related to your treatment of diabetes:
Indication for use:
The FreeStyle Libre Flash Glucose Monitoring System is indicated for measuring interstitial fluid glucose levels in people (age 4 and older) with diabetes mellitus. The indication for children (age 4 - 17) is limited to those who are supervised by a caregiver who is at least 18 years of age.
It is designed to replace blood glucose testing in the self-management of diabetes with the exceptions listed below. Under the following circumstances, use a blood glucose meter to check the current glucose readings from the FreeStyle Libre Flash Glucose Monitoring System Sensor.
Conditions when a blood glucose test with finger prick is required:
- During times of rapidly changing glucose levels, interstitial glucose levels as measured by the sensor and reported as current may not accurately reflect blood glucose levels. When glucose levels are falling rapidly, glucose readings from the Sensor may be higher than blood glucose levels. Conversely when glucose levels are rising rapidly, glucose readings from the sensor may be lower than blood glucose levels.
- In order to confirm hypoglycemia or impending hypoglycemia as reported by the sensor.
- If symptoms do not match the FreeStyle Libre Flash Glucose Monitoring System reading. Do not ignore symptoms that may be due to low blood glucose or high blood glucose.
