Subscription Terms & Conditions

1. The FreeStyle Libre Subscription Plan is for deliveries to Singapore addresses only.

2. The FreeStyle Libre Subscription Plan requires a valid credit card to be securely saved to your account profile. This credit card will be charged upon your next upcoming order date. Orders will not be accepted, fulfilled or shipped if the credit card fails for any reason whatsoever, including expiration and insufficient funds. Your subscription will be cancelled if we are unable to charge your credit card after 7 attempts over 7 days.

3. An order will be placed for you automatically every 4 or 8 weeks (depending on the subscription plan option chosen) unless you choose to change, pause or cancel your subscription plan. For example, if you made your first order on 1 January 2024, your next order will be on 29 January 2024. Your subscription will continue unless it is cancelled.

4. Your credit card will automatically be charged when your product is delivered for subsequent purchases after the first purchase, unless you choose to change, pause or cancel your subscription plan.

5. You may have more than 1 subscription plan. Should you choose to upgrade or downgrade your existing plan, please cancel your existing plan and add the new plan to cart before checking out.

6. For subscription plan cancellations, you must cancel your plan at least 2 working days before your next order occurrence date. Cancellations or changes will take effect on the next order occurrence cycle. Please refer to the subscription FAQ above on how you can cancel or change your subscription plan.

7. Every 6th order is free and will be delivered 4 weeks or 8 weeks (depending on the subscription plan option chosen) after the 5th order.

8. The equivalent value of a non-subscription 6 order cycle is $1,291.20 (12 sensors at $103.10 each, plus shipping costs of $9 each for six orders). This represents an actual saving of about 20%.

9. The offer is only available online. Not valid for use in conjunction with other FreeStyle Libre promotions or offers.

10. Abbott reserves the right to change pricing and offers at any time. Subscribers will be notified of such changes at least 1 billing cycle in advance through email notifications.

11. Except for any changes to offers and pricing set out in point #8 above where prior notice will be given to you, Abbott reserves the sole and absolute right to withdraw, add or vary any term relating to your subscription from time to time without giving any prior notice to you. You will be bound by the terms and conditions so amended.

12. An email reminder will be sent to you 7 days before your next order occurrence date. You will receive your electronic bill via email after your order has been automatically placed and your credit card will automatically be charged when your product has been delivered to you.

13. You agree that Abbott's decision on all matters or disputes relating to your subscription and any matters ancillary thereto (including without limitation payment, delivery, promotions or eligibility requirements) are final and conclusive and binding on you.

14. You hereby agree and consent to:

• a) Abbott using and disclosing your personal data for the purposes of processing your subscription application and to provide you with the products you have requested; and
• b) The terms of our Privacy Policy at https://freestylelibre.com.sg/privacy-policy as amended from time to time and the terms of our Privacy Policy are incorporated in the Terms and Conditions for the FreeStyle Libre Subscription Plan by reference.
